Our Programs

Recovery

The ‘Recovery Program’ commences with a face to face assessment, allowing a care plan to be written.  This encompasses all aspects of a person’s life, including their dreams and goals for the future. To fulfill the plan, recovery staff are allocated to a person to ensure their needs are met, whether that be education, life skills, preparation for employment, meaningful occupation,  health, promoting positive relationships and/or socialising.

Family and Carer

The ‘Carer Program’ commences with a face to face assessment which utilises MI-CAT (Mental Illness Carer Assessment Tool) to develop a care plan.

We acknowledge that carers are a key member of a person’s recovery team.  We understand the importance of providing support and advice to carers.

Individual and group sessions are available with lived carer experienced facilitators.

Our service covers many areas in the South West , face to face ,over the telephone and educational programs in person or via ZOOM

Supported Housing

Pathways SouthWest is a community housing provider; we maintain a number of houses dedicated to those with a lived or living experience of mental health issues.

As a landlord, we focus on reducing many of the tensions created by difficulties in finding accommodation and maintaining properties in good order.
